Program Description
C.O.A.C.H. Mental Health, LLC Psychiatric Rehabilitation Program (PRP) provides community based and individualized support to Children and Adults in need of additional behavioral health support to address areas of functioning that may be impaired or that require rehabilitation. As a mental health provider assesses a client’s needs for additional services, they may then refer the client for PRP. In collaboration with the referring licensed mental health provider, PRP serves to support the child or adult’s mental health goals and objectives.
Through our evidence-based, comprehensive, individualized and need-based rehabilitation services, we promote holistic development for children and adults with persistent and severe mental health illnesses. While offering evidence-based education and support, and individualized rehabilitation planning, we also provide individualized, need-based intervention. We encourage our clients, their guardians and support systems to assume ownership and an active role in development and implementation of service goals, activities, services and interventions.
In order to support the long-term success, given the complexity of our clients’ circumstances, C.O.A.C.H. employs an innovative system of service provision, at our facility (On-Site) or in the community (Off-Site). Our comprehensive assessment/evaluation, in conjunction with information from the referring mental health professional and the client and family allow us to develop specific rehabilitation goals and objectives.
Referring Mental Health clinicians, guardians and other pertinent stakeholders are regularly provided updates regarding each client’s progress based on ongoing evaluations from C.O.A.C.H. Mental Health, LLC staff.
